If you are interested in having your child receive the Flu vaccination at school, please download the 2014-15 Flu Vaccine Screening and Consent Form available below and return the completed form to the School Nurse BEFORE the date listed above.
Family members can also receive a free flu vaccine at the school site, before or after school only, on the dates listed above (please take completed forms with you). After these dates, flu vaccines will be offered on a first-come, first-serve basis at the CCMS/HS Nurse’s Office while supplies last or until December 15, 2014, whichever occurs first.
